ok got that part fixed though now it tells me it cannot load gamemd.exe when it starts up I've just gone and got my vanilla YR gamemd after uninstalling npatch so i'm not sure whats wrong here

Yea your were right I did have that hide extensions checked I fixed that and it runs as a CMD but when I run it it gives me that syringe cannot just run by itself message

I probably missed something I extracted the update files from this thread added the Visual C++ update but it did'nt do anything when I dragged the GAMEMD.syr to syringe just kinda looked like it loade...
